Home Health Visiting Nurse - LVN

• Does skilled nursing care at patients homes 
• Does wound care, IV therapy, medication mgmt, diabetic management, etc
• Follows the Plan of Care and notifies Clinical Supervisor of changes in orders.
• Delivers compassionate, accurate, quality care
• Produces clear, concise, and accurate documentation and submits within required time frames.
• Recognizes deviations in patient conditions and makes appropriate notifications
• Displays successful decision-making and problem solving skills, and coordinates with the Clinical Supervisor when appropriate
• Establishes and sustains a professional rapport with patients, families, co-workers, and SSHS leadership
• Maintains effective communication and coordination between SSHS staff, patients and families
• Exercises effective conflict avoidance and resolution skills
• Maintains current knowledge of work topics, including new diagnoses and medications
• Conforms with all SSHS policies
• Minimizes call-outs
• Arrives on time to work and displays a strong work ethic
• Supports team-building efforts
• Displays strong organizational and time-management skills
• Continually strives for quality improvement

Requirements

• 1 year of nursing experience/ new graduate will be trained
• Valid DL/State License
• Active Registered Nurse License
• CPR
• TB
