Responsibilities

• Provide direct care to residents, including assistance with ADLs, mobility, and transfers

• Observe and report changes in residents' conditions to the nursing staff and medical team

• Maintain a clean and safe environment for residents, including assisting with laundry and housekeeping tasks

• Work effectively as part of a multidisciplinary team to achieve patient-centered care goals

• Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination
