Admit-to-Discharge Therapist – PRN
$55/Hour | Outpatient + Home Care | Flexible Weekday Schedule
Community Physical Therapy (CPT) is looking for a PRN therapist to join our Admit-to-Discharge team! If you’re looking for flexibility, a strong team environment, and the opportunity to work with patients from evaluation through discharge, this could be a great fit.
What makes this opportunity stand out:
$55/hour PRN rate
Monday–Friday flexibility, with scheduling between 7am–6pm
Outpatient + home care patients
Follow your patients from evaluation through discharge
Ability to build a caseload that works for you
Support from a therapist-led organization

What we’re looking for:
Active Illinois therapy license
Outpatient or home care experience is a plus
Someone dependable, flexible, and comfortable working independently
Willingness to learn TheraOffice and Homecare Homebase
