Physical Therapist Job Summary
We are seeking a dedicated Physical Therapist to provide exceptional care to patients in their homes who are experiencing physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. The patient demographic ranges from infants to seniors over 100 years of age.
Key Responsibilities- Deliver physical therapy services tailored to individual patient needs.
- Collaborate effectively with patients, their families, physicians, and other allied health professionals to determine patient needs and coordinate services accordingly.
- Utilize thorough and timely electronic documentation and interdisciplinary communications in alignment with agency policies, procedures, and standards of practice.
- Must hold a valid Physical Therapist Professional license in Massachusetts.
- Minimum of 1 year of experience in home care is required.
- 1 year of case management experience is also required.
- Experience in hospice or private care does not satisfy the requirements for certified home health care management experience.
- Must be able to travel to assigned cases as needed.
- Weekend rotations are required once every six weeks.
The start date for this position will be determined based on credentialing clearance. If credentialing is not completed by the scheduled start date, a rescheduled start date will be necessary.
