Position Summary:
We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager(s) to join our hospice care team in the ABQ, and surrounding areas. The RN Case Manager is responsible for the overall coordination of care for hospice patients, ensuring the delivery of high-quality care in accordance with the patient’s individualized care plan.

Key Responsibilities:

• Provide skilled nursing care and assessments to hospice patients in their homes or facility settings
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Communicate regularly with physicians, patients, families, and the hospice care team to coordinate care
• Educate patients and families on hospice philosophy, symptom management, and medication usage
• Participate in on-call rotation to support patient needs after hours and weekends.
• Maintain accurate and timely documentation in accordance with regulatory standards

Qualifications:

• Current and unrestricted RN license.
• Minimum of 1 year of nursing experience; hospice or palliative care experience preferred
• Strong communication, organizational, and clinical assessment skills
• Valid driver’s license and reliable transportation
