Job Description
The Intake Coordinator is responsible for managing incoming referrals and information calls in a prompt, professional, and courteous manner. This position works closely with patients, families, referral sources, and internal teams to identify service needs, coordinate assessments and admissions, and ensure accurate and timely documentation.
Schedule: Monday - Friday, 8:00AM - 4:30PM
Responsibilities:
Answer and manage incoming calls, directing non-referral calls to the appropriate staff members
Manage incoming referrals and provide information regarding available programs and services
Schedule assessment and admission appointments with appropriate staff
Document calls, referrals, appointments, and follow-up activity accurately
Follow up on pending referrals with medical providers, family members, and other appropriate contacts
Coordinate with outside organizations to obtain necessary medical records
Communicate with internal teams and referral sources to effectively manage workflow
Coordinate and document face-to-face scheduling needs for pending referrals
Maintain accurate patient and family information, including addresses, locations, and other updates
Prepare and provide daily status reports to appropriate team members
Maintain positive relationships with internal and external customers
Assist leadership with administrative tasks and special projects as needed
Maintain working knowledge of available programs, services, and community resources
Perform additional duties as assigned

Requirements:
High school diploma required
Associate"s or bachelor"s degree, or equivalent experience, preferred
Previous telephone-based customer service experience
Ability to effectively manage a high volume of incoming calls
Excellent telephone and interpersonal customer service skills
Strong verbal and written communication skills
Ability to appropriately engage and disengage callers
Strong organizational skills with the ability to prioritize and manage multiple responsibilities
Ability to work independently and interact effectively with individuals at all levels of the organization
Proficiency with computers and Microsoft Office programs
Minimum typing speed of 40 WPM
Ability to maintain accuracy and attention to detail while documenting information and managing follow-up activities
Ability to work for extended periods at a computer and perform repetitive motions
