Physical Therapist (PT) – Home Health

Key Responsibilities- Provide skilled physical therapy services to a geriatric home health patient population
- Conduct and complete OASIS assessments, including start of care evaluations
- Manage a caseload of up to 6 patient visits per day, meeting a standard of 30 productivity points
- Document all clinical activity accurately and timely using HCHB (HomeCare HomeBase) EMR
- Utilize personal transportation to travel to patient homes; service area may occasionally extend up to 50+ miles
- Active California Physical Therapist (PT) license required
- Basic Cardiac Life Support (BCLS) certification required
- Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
- Proficiency with HCHB EMR required
- OASIS experience required
- Must have reliable personal transportation
- Experience working with geriatric patient populations in a home health setting
- Strong time management skills with the ability to efficiently manage a multi-patient daily caseload
- Caseload is geographically grouped to minimize drive time and maximize patient care time
- 1:1 patient care model allowing for focused, individualized therapy delivery
