Occupational Therapist (OT) – Home Health

Key Responsibilities- Provide occupational therapy services to patients in a home health setting across the Oneida and Vilas County territory
- Manage a caseload spanning coverage areas including the Rhinelander and Arbor Vitae regions
- Document patient care and progress using HCHB (HomeCare HomeBase) software
- Develop and implement individualized treatment plans to support patient functional independence
- Active Occupational Therapist (OT) license in the state of Wisconsin
- Previous home health experience required
- Proficiency with HCHB (HomeCare HomeBase) documentation system required
- Valid driver's license and reliable transportation for home visits across assigned territory
- Familiarity with rural or multi-county territory coverage
- Strong time management and self-directed work skills suited for a home-based caseload
- No on-call or weekend expectations, offering a consistent and predictable Monday–Friday schedule
