Job Summary

We're seeking a Per Diem Physical Therapist Assistant (PTA) to join our dedicated rehabilitation team at Rock River Nursing & Rehab in Fort Atkinson, WI. The successful candidate will assist our physical therapists in providing high-quality patient care, while ensuring a positive and respectful environment for our patients and colleagues.

Responsibilities

• Assist physical therapists in the implementation of individualized treatment plans, including exercises, stretches, and other interventions

• Conduct patient assessments and document outcomes in accordance with department policies and procedures

• Collaborate with the rehabilitation team to develop and progress patient care plans

• Provide education and support to patients, families, and caregivers on a variety of topics related to physical therapy

• Maintain accurate and thorough patient records, ensuring compliance with regulatory requirements

• Participate in team meetings and contribute to the development of departmental policies and procedures

Requirements

• Current certification as a Physical Therapist Assistant (PTA) in the state of Wisconsin

• High school diploma or equivalent required; associate's degree or higher in physical therapy assistant studies or related field preferred

• Per Diem schedule; flexible availability required
