Job Summary:

We are seeking a highly motivated and skilled Certified Occupational Therapy Assistant (COTA) to join our team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin. As a COTA, you will play a vital role in providing high-quality care to our residents, utilizing your expertise to develop and implement individualized occupational therapy plans. Our Per Diem COTA will work alongside our dedicated healthcare team to promote recovery, independence, and overall well-being.

Responsibilities:

• Assist Occupational Therapists in the development and implementation of occupational therapy treatment plans

• Conduct therapy sessions with residents, focusing on activities of daily living (ADLs), mobility, and cognitive skills

• Monitor and record patient progress, providing recommendations for adaptations or modifications as needed

• Collaborate with other healthcare professionals to ensure comprehensive care and communication

• Participate in interdisciplinary team meetings to discuss patient care and treatment plans

Requirements:

• Current certification as a Certified Occupational Therapy Assistant (COTA) in the state of Wisconsin

• High school diploma or equivalent required; associate's degree or higher in Occupational Therapy Assistant preferred

• Experience in a long-term care or rehabilitation setting preferred

• Strong communication and interpersonal skills
