Social Worker

Capstone is looking for a Social  Worker to help organize and plan the Social Service program for residents based on their individualized needs within the facility.  

Job Depscription- Social Worker

• Regularly assess the social and emotional needs of the resident, sets appropriate goals for meeting those needs within the overall plan of care and provides ongoing and regular services to meet the needs and achieve goals.

• Serves as primary liaison between residents/families and staff as it relates to any all provided services for residents.

• Responds to referrals initiated by other departments in a timely manner.

• Completes all documentation timely.

• Completes Social Service section of the Minimum Data Set.

• Participates in the interdisciplinary care plan meetings and invites the resident and family to attend.  

• Provides consultation to other departments regarding individual resident or general situations that may affect the social and emotional well-being of residents. 

• Assists with burial arrangements if no family is available.

• Attends weekly department meetings

• Other duties as requested

Requirements-Social Worker:

• Bachelors degree in a Human Service Field 

• Experience of 1 year in a long term care setting (preferred)

• Able to communicate and interact with all members of the health care team

• Preferred experience as a social worker
