Blize Healthcare is seeking a detail-oriented and compassionate Intake Coordinator to join our team. This role is essential in ensuring patients experience a smooth, supportive transition into home health and hospice care.
Responsibilities:
- Manage daily patient referral and intake operations.
- Verify insurance coverage and assist the billing department with claim processing.
- Complete insurance forms and respond to patient inquiries.
- Schedule home health and hospice visits efficiently.
- Answer, screen, and direct phone calls; take messages; schedule appointments.
- Greet and assist visitors in a professional manner.
- Receive, sort, and distribute mail, documents, and packages.
- Maintain medical supply inventory.
- Select and package DME supplies.
- Support colleagues with general administrative tasks.
- Maintain Patient Record.
- Send Orders needed to sign to physicians and follow up with physicians.
- Attach patient records and consent forms in the company system.
Requirements:
- Minimum 1 year of experience in the medical field.
- Strong verbal communication skills.
- Excellent interpersonal and customer service skills.
- Basic understanding of administrative and clerical procedures.
- Proficiency in Microsoft Office (Word, Excel, Outlook) and Google Workspace (Docs, Sheets).
