Rate: $19.00 - $22.00/hr. (depending on experience)

Absolut Care of Aurora Park is seeking a Social Worker to join our Long-Term Care team in East Aurora, NY. This role is key in supporting residents and families while ensuring compliance with state and federal LTC regulations.

Key Responsibilities:

• Provide psychosocial assessments and ongoing support for LTC residents

• Assist residents and families with care planning, adjustment, and discharge needs

• Coordinate with interdisciplinary team, nursing, admissions, and community resources

• Support care conferences, resident rights, and regulatory documentation

• Advocate for residents’ emotional, social, and psychosocial well-being

Qualifications:

• Degree in Social Work 

• Experience in Long Term Care / Skilled Nursing strongly preferred

• Knowledge of NYS DOH and federal LTC regulations

• Strong communication, documentation, and advocacy skills
