We are seeking a Physician (MD/DO) to serve as a contracted Medical Director in long-term care (LTC), skilled nursing, and nursing home settings. In this role, you will partner with facility leadership, NPs/PAs, and nursing teams to support high-quality, compliant, and resident-centered care.
Key Responsibilities
- Participate in monthly or quarterly QAPI meetings and provide guidance on quality and clinical outcomes.
- Review key metrics, including readmissions, falls, infections, and antipsychotic utilization.
- Support infection prevention and antibiotic stewardship initiatives.
- Collaborate with NPs, PAs, the Director of Nursing, and facility leadership to ensure continuity of care and effective care transitions.
- Monitor adherence to resident care policies and clinical standards.
- Assist with survey readiness, regulatory compliance, and corrective action planning.
- Provide clinical mentorship and support for complex medical and behavioral cases.

Typical Time Commitment
- 4 hours per month per facility for administrative duties
- On-site presence expectations once weekly for rounding on acute care and long-term care patients. Approx. 2 hours per week and may include:
- Primary and post-acute care for patients in the facility
- Managing chronic conditions (CHF, COPD, dementia, diabetes)
- Supporting advanced practice providers (NPs/PAs), including clinical guidance and collaboration
- Participation in QAPI and leadership meetings
- Care planning, interdisciplinary team (IDT) meetings, and quality initiatives
- Availability for consultation
Qualifications
- MD or DO with active state medical license
- Board certification in geriatric, family or internal medicine
- Experience in geriatrics, nursing homes, post-acute, or long-term care
- Knowledge of CMS and LTC regulatory requirements
- Strong leadership and provider oversight capabilities
- Excellent relationship-building and teamwork skills
- Effective communication with interdisciplinary teams and elderly patients
- Commitment to quality, value-based care, and geriatric outcomes
