Shift: Day | 5x8 Hours | 8:00 AM–4:30 PM
Schedule: Every other weekend

Job Overview

We are hiring an experienced Registered Nurse (RN) Case Manager for an acute care hospital in Burbank, CA.

Requirements

• Active CA RN License required
• Minimum 2 years of acute care Case Management experience
• Recent experience with Care Coordination and Discharge Planning
• Epic charting experience required
• Case Management / Utilization Review experience
• Experience with patient assessments and determining medical necessity
• Experience with HMOs, IPAs, and healthcare regulations
• Travel experience required; first-time travelers may be considered

Responsibilities

• Coordinate patient care and develop individualized plans of care
• Perform discharge planning and care coordination
• Assess patient needs and arrange DME/services
• Determine medical necessity using evidence-based guidelines
• Collaborate with interdisciplinary healthcare teams
• Maintain accurate electronic documentation

• Patient population: Adults & Geriatrics
• Caseload: Up to 25 patients
• Ratio: 1:25
• No floating required
• 40 hours of orientation
• Business attire or scrubs
