Mental Health Case Manager, St. Louis

A non-profit organization dedicated to combating homelessness is seeking a mission-driven LCSW to join their program. This is a full-time role, Mon-Fri 8-4:30. Length of assignment is flexible; temporary, temp-to-perm, or direct hire.

Key Responsibilities:

• Conduct comprehensive, ongoing assessments of participants' mental illness, physical health, housing status, and other relevant needs.

• Provide direct clinical services to clients on an individual and group basis, operating both in the office and in community settings.

• Facilitate treatment planning groups and collaborate with clients to set goals focused on personal growth, wellness, and recovery.

• Consult with and act as a liaison to community agencies, providing appropriate referrals and maintaining coordination in the treatment process
