Axia Care Center is seeking an experienced Director of Social Services to lead our Social Services Department.

? SKILLED-NURSING EXPERIENCE IS REQUIRED

Candidates must have:

• At least two years of recent Social Services experience in a skilled nursing or long-term-care facility

• Active New Jersey LSW or CSW

• Hands-on experience with MDS documentation

• Hands-on experience completing and managing POLST forms

• Experience investigating, documenting, and resolving resident and family grievances

• Experience with care plans, discharge planning, advance directives, resident rights, and regulatory compliance

• Previous leadership or supervisory experience preferred

Key Responsibilities:

• Direct the daily operations of the Social Services Department

• Complete required MDS documentation accurately and on time

• Manage POLST forms, advance directives, and healthcare decision-making documentation

• Investigate and follow up on resident and family grievances

• Participate in care-plan and interdisciplinary team meetings

• Coordinate safe discharge planning and community services

• Address psychosocial, behavioral-health, and resident-rights concerns

• Maintain compliance with federal and New Jersey skilled-nursing regulations

• Supervise Social Services staff and ensure timely documentation
