Responsibilities:

• Provide direct patient care, including personal care, vital signs, and medication assistance, in accordance with established nursing standards and protocols

• Develop and maintain a positive and supportive environment for residents, families, and staff

• Collaborate with interdisciplinary teams to develop and implement individualized care plans

• Assists with meals, laundry, and other daily living activities as needed

• Participate in quality improvement initiatives to enhance resident care and satisfaction

• Maintain accurate and timely records of patient care and services provided

Requirements:

• Current Certified Nursing Assistant (CNA) certification in the state of Pennsylvania

• High school diploma or equivalent required

• Ability to lift, transfer, and move patients, as well as stand and walk for extended periods

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment with frequent interruptions

• Reliable transportation and a valid driver's license
