Occupational Therapist

Occupational Therapist (Contract)
Location: Arizona 86404
Pay: $1,987 – $2,230/week

• 5 shifts per week, 8 hours each (flexible: 4/10s or 5/8s)

• Must work until 6:00 PM MST

• 26-week commitment preferred; permanent possible

• Provide occupational therapy evaluations and treatment for patients

• Maintain accurate daily documentation using DSCTop and EIEPPRO (for annuals, evaluations, quarterlies)

• Collaborate with interdisciplinary team and follow organizational documentation standards

• Support patient progress and care planning while adhering to regulatory and safety guidelines

• Occupational Therapist license (OT, not COTA) – Required

• BLS certification – Required

• Experience in patient evaluations and treatment – Preferred

• Flexible with schedule and able to work until 6:00 PM MST
