Occupational Therapist (OT) - Hope Creek Nursing and Rehabilitation Center

As an Occupational Therapist (OT) on our Per Diem team, you will have the opportunity to work in a dynamic and supportive environment, providing high-quality care to individuals in our skilled nursing facility.
About the Role:
As an Occupational Therapist, you will be responsible for evaluating, planning, and implementing individualized occupational therapy programs to help patients achieve their maximum functional potential. You will work closely with our interdisciplinary team to develop and implement treatment plans, provide individual and group therapy sessions, and educate patients and their families on home programming and maintenance. Your expertise and compassion will help patients recover from injuries, manage chronic conditions, and regain independence.
Responsibilities:
• Evaluate patients' physical and cognitive abilities and develop treatment plans to address their specific needs
• Implement and modify occupational therapy programs to promote patient progress and achievement of goals
• Provide individual and group therapy sessions, including exercises, activities, and education
• Collaborate with other healthcare professionals to develop and implement comprehensive treatment plans
• Educate patients and their families on home programming and maintenance to support continued progress
• Document patient progress and treatment plans in a timely and accurate manner
Requirements:
Bachelor's or Master's degree in Occupational Therapy from an accredited program
Current state licensure and certification as an Occupational Therapist (OT)
Ability to pass a background check and comply with our facility's policies and procedures
