Registered Nurse Home Health

We are looking for a Registered Nurse with home health experience to join our team and provide quality nursing support to clients in their homes.

Responsibilities:

• Provide nursing care and support to clients in a home care setting.
• Assess client needs and monitor changes in condition.
• Develop and follow appropriate care plans based on client needs.
• Communicate with clients, families, caregivers, and the care team regarding client care.
• Complete accurate and timely clinical documentation.
• Monitor and report changes in client condition to the appropriate care team.
• Coordinate with caregivers and other healthcare professionals as needed.
• Follow applicable clinical, safety, and documentation requirements.
• Maintain client confidentiality and professional standards at all times.

Qualifications:

• Current and valid Registered Nurse (RN) license.
• Previous home health experience required.
• Strong clinical assessment and communication skills.
• Ability to work independently in a home care environment.
• Strong documentation and organizational skills.
• Ability to communicate effectively with clients, families, caregivers, and healthcare professionals.
• Reliable and professional approach to client care.

Preferred:

• Experience working with home care or home health clients.
• Familiarity with care plans, clinical documentation, and coordination of in-home services.
