Registered Nurse

Registered Nurse – Home Health

We are seeking an experienced Registered Nurse (RN) to provide high-quality home health care and case management to patients throughout the community. The RN will assess patient needs, develop and implement care plans, coordinate with healthcare providers, and provide skilled nursing services.

Position Details

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• On-Call: 1 weekend/month plus occasional weekday after-hours coverage
• License: Active RN license
• Mileage: $0.49/mile
• Coverage Area: Jackson and Barrow Counties

Responsibilities

• Assess, plan, implement, and evaluate patient care plans.
• Provide skilled nursing care for chronic and acutely ill patients.
• Monitor patient health status and report changes to physicians.
• Coordinate care with physicians, nurses, social workers, and other healthcare providers.
• Manage medications, physician orders, and required clinical documentation.
• Coordinate appointments, hospitalizations, discharges, and transitions of care.
• Supervise LPN/LVN staff and provide training to nursing and unlicensed personnel as needed.
• Educate staff and patients on health maintenance, safety, and disease prevention.
• Maintain accurate nursing documentation and ensure compliance with applicable policies and procedures.
• Support infection prevention through proper maintenance of equipment, supplies, and patient environments.

• Home health experience preferred

Qualifications

• Current, unrestricted RN license.
• Home health nursing experience strongly preferred.
• Strong clinical assessment, organization, communication, and care coordination skills.
• Ability to work independently and effectively collaborate with multidisciplinary teams.
