Registered Nurse – Case Management

We are seeking a Registered Nurse (RN) to provide comprehensive case management and care coordination for high-risk members. This role focuses on developing individualized care plans, coordinating services, and helping patients achieve optimal clinical and functional outcomes.

Position Details

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Guaranteed Hours: 40 hours/week
• On-Call: Saturday/Sunday SOC/ROCs only
• License: South Carolina RN license or Compact license accepted
• Productivity: 30 points/week (6 points/day)
• Mileage: $0.49/mile
• Coverage Area: Florence, Darlington, Williamsburg, Marion, Dillon, and Marlboro Counties

Responsibilities

• Perform comprehensive assessments of high-risk patients and continuously reassess their care needs.
• Develop and implement individualized care plans in collaboration with providers.
• Coordinate transitions of care with physicians, nurses, social workers, discharge planners, and other healthcare professionals.
• Identify appropriate, cost-effective levels of care and connect patients with needed resources such as housing and transportation.
• Identify drug, clinical, and care-related concerns and coordinate appropriate interventions.
• Promote health, prevent illness, and minimize adverse patient outcomes.
• Maintain detailed clinical, functional, and fiscal documentation.
• Advocate for patients while protecting privacy and confidentiality.

Qualifications

• Associate’s or Bachelor’s degree in Nursing or a related field.
• Current, unrestricted SC RN license or Compact license.
• Case Management certification required.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Ability to analyze complex medical information and collaborate effectively with multidisciplinary teams.
