About the Role:

As an LPN/RN at Orrville Skilled Nursing and Rehabilitation, you will be responsible for:

• Providing direct patient care, including administering medications, conducting vital signs, and performing various medical procedures

• Developing and implementing individualized care plans tailored to each guest's unique needs and goals

• Collaborating with other healthcare professionals to ensure high-quality, patient-centered care

• Maintaining accurate and detailed records of patient care and progress

• Providing emotional support and education to guests and their families

Responsibilities:

• Provide nursing care to guests in accordance with established policies and procedures

• Conduct initial and ongoing assessments of guests' physical and emotional needs

• Develop and implement individualized care plans, in conjunction with other healthcare professionals

• Monitor and report changes in guests' conditions to healthcare professionals and family members

• Participate in multidisciplinary care conferences to discuss patient progress and goals

• Maintain accurate and detailed records of patient care and progress
