QUALIFICATIONS:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Assesses patient status by obtaining health history through patient/family interviews and chart reviews, assessing presenting illness, risk factors, family history, psychosocial situation and performing appropriate physical exam.
  • Performs scheduled peer to peers with insurance payers during non-patient care moments.
  • Orders appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation and coordination with physicians and clinicians, for evaluation of care.
  • Follows up with obtaining lab and diagnostic results and notifying collaborative physician of any significant results.
  • Educates patients regarding balanced treatment options
  • Develops care plan for opioid therapy in accordance with state and federal opioid prescribing guidelines and collaborative agreement with pain management provider
  • Works collaboratively with physicians, and other healthcare providers to optimize patient care
  • Current Indiana nursing license with renewal required.
  • A Master of Science Degree in Nursing preferred.
  • Professional knowledge of nursing theory and practice.
  • Extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, and patient care management.
  •  Maintains CCU's for certification renewal.

MANDATORY LICENSE/REGISTRATION/CERTIFICATION:

 The following licenses/certifications are required (if checked): 

CPR (Cardio-Pulmonary Resuscitation)
ACLS (Advanced Cardiac Life Support)
PALS (Pediatric Advanced Life Support)
NRP (Neonatal Resuscitation Program)

 

CERTIFICATION/LICENSE:  Nurse Practitioner (NP); Registered Nurse (RN)

 

ADDITIONAL LICENSURE/CREDENTIAL REQUIREMENTS:

  • Current State of Indiana NP License with prescriptive privileges.
  •  National NP certification required

 

ADDITIONAL EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Mandatory Continuing Education: Customer Service, Fire and Safety, Corporate Compliance (including Confidentiality), Infection Control, and education required by regulatory, accreditation bodies, scope of practice, and/or Hancock Regional Hospital.
  • Demonstrated competence in area of clinical specialization.
  • Ability to access, review, and synthesize clinical information from multiple sources to make clinical decisions
  •  Ability to utilize and explain risk stratification tools to patients and their families.
