About the Role:

As a Licensed Social Worker at Avir at Hearne, your primary responsibility will be to assess and address the psycho-social needs of our residents. You will work closely with our interdisciplinary team to develop and implement comprehensive care plans, providing individualized support and services to promote a high quality of life. Your expertise will also be utilized to facilitate group therapy sessions, family support sessions, and other social activities to foster a sense of community and overall well-being.

Responsibilities:

• Conduct psychosocial assessments and develop individualized care plans to address the physical, emotional, and social needs of our residents
• Provide individual and group therapy sessions to support residents' mental health and well-being
• Facilitate family support sessions and other social activities to enhance the overall quality of life for our residents
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans
• Monitor and evaluate the effectiveness of care plans, making adjustments as needed
• Maintain accurate and timely records of resident interactions and assessments

Requirements:

• Licensed Social Worker (LMSW or LCSW) in the State of Texas
• Bachelor's degree in Social Work or related field
• Strong interpersonal and communication skills
• Ability to work effectively in a fast-paced environment
• Passion for delivering exceptional patient-centered care
