Job Summary

Homeside Rehab is seeking compassionate and skilled Speech-Language Pathologists (SLPs) to join our growing home health team throughout Brooklyn, NY.

In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility, consistent case opportunities, and support from our clinical and administrative teams.

Current Brooklyn Areas Include:

• East New York – 11207, 11208

• Canarsie – 11236

• Flatbush – 11226

• East Flatbush – 11203

• Crown Heights – 11213

• Brownsville – 11212

• Bushwick – 11221

• Bedford-Stuyvesant – 11216

• Brighton Beach / Sheepshead Bay – 11235

• Additional Brooklyn locations available based on patient needs

Requirements

• Active New York State Speech-Language Pathologist License – Required

• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred

• Basic computer literacy for documentation and communication

• Strong clinical and interpersonal communication skills

• Ability to work independently in a home health environment

• Reliable transportation for home visits preferred

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on patient needs and goals

• Provide evidence-based treatment for speech, language, fluency, articulation, voice, and related communication disorders

• Maintain accurate and timely documentation of patient care

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Communicate effectively with office staff, patients, caregivers, and families

• Collaborate with other healthcare professionals to support comprehensive patient care

• Location: Brooklyn, NY – Multiple neighborhoods and ZIP codes

• Schedule: Flexible scheduling with day and evening availability

• Monday through Friday

• Weekend availability as needed

• Choose cases and schedules based on availability
