  • Location: Hiawatha & Cedar Rapids, IA (On-site)
  • Employment Type: Permanent, Full-Time
  • Schedule: Tuesday – Saturday, 8:30 AM – 5:00 PM


Position Summary

We are seeking a customer-focused Hospital Liaison to work directly within a hospital environment in the Hiawatha and Cedar Rapids area. This role serves as the crucial connection between hospital discharge teams and durable medical equipment (DME) / respiratory care services.

You will work directly alongside case managers, social workers, nurses, physicians, patients, and caregivers to ensure patients have the equipment, education, and support required for a safe transition home.


Key Responsibilities

  • Discharge Coordination: Collaborate with hospital case managers, social workers, nurses, and discharge planners to coordinate respiratory and DME services for outgoing patients.
  • Patient & Caregiver Education: Educate patients and caregivers on proper equipment operation, safety, and routine maintenance prior to leaving the facility.
  • Equipment & Logistical Support: Ensure timely delivery and setup of respiratory and durable medical equipment; resolve urgent equipment needs and troubleshoot basic service concerns.
  • Administrative & Insurance Documentation: Maintain accurate referral forms, insurance verification documentation, delivery tickets, and activity records.
  • Relationship Management: Serve as a trusted resource for hospital personnel regarding available services, representing the organization professionally at hospital meetings and healthcare events.
  • Field Visits (Occasional): Travel to patient homes for equipment setup and follow-up when required.

Required Qualifications

  • High school diploma or GED required.
  • Valid driver’s license with a clean driving record.
  • Ability to safely lift, load, and transport equipment weighing up to 65 pounds.
  • Compliance with applicable hospital safety, infection control, and respiratory protection requirements.

Preferred Qualifications & Skills

  • Experience in hospital, clinic, DME/home medical equipment, patient service, or healthcare coordination settings.
  • Familiarity with medical terminology, medical records, and insurance reimbursement processes.
  • Excellent customer service, relationship-building, and interpersonal communication skills.
  • Ability to manage multiple patient discharge needs simultaneously in a fast-paced environment.
  • Proficiency with Microsoft Office (Outlook, Word, Excel) and electronic health record systems.
