We are looking for a Care Coordinator to support care management operations and help ensure members receive timely, well-organized services in Syracuse, New York. This contract opportunity with potential for a permanent role is ideal for someone who enjoys combining administrative accuracy with member support in a healthcare setting. The person in this role will work closely with clinical staff to manage referrals, track authorizations, coordinate service delivery, and maintain clear communication across teams and external partners.

Responsibilities:
• Enter and maintain authorization information in internal systems with a high level of accuracy, and monitor upcoming expirations to support uninterrupted services.
• Coordinate approved services with community providers, agencies, and internal team members to help meet member care needs efficiently.
• Support case management activities by arranging appointments, organizing service-related tasks, and assisting with day-to-day care coordination priorities.
• Partner with the clinical team in regular discussions to identify workflow challenges and suggest practical process improvements.
• Prepare, update, and distribute operational reports as requested to help track performance and support departmental needs.
• Provide general administrative support through document handling, correspondence, record maintenance, message management, and other office-related tasks.
• Communicate routinely with members to provide updates, gather needed information, and help address service-related questions.
• Contribute to quality and performance improvement efforts by following established procedures and supporting ongoing operational excellence initiatives.• High school diploma required; an associate degree in a related field is preferred.
• At least 2 years of experience in healthcare administration, care coordination, medical support, or a similar environment; medical assistant training may also be considered.
• Strong computer proficiency with the ability to learn new systems quickly and use healthcare technology effectively.
• Working knowledge of Microsoft Office applications, internet-based tools, and data entry processes.
• Demonstrated analytical thinking and problem-solving ability, with the capacity to recognize issues and support resolution efforts.
• Ability to manage responsibilities independently while also collaborating effectively with cross-functional team members in a changing environment.
• Sound judgment and discretion when working with confidential or sensitive information.
• Strong verbal and written communication skills, along with customer service and scheduling experience.
