Job Description
Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) for a 3-month contract assignment. In this field-based role, you will travel throughout the community to conduct health assessments, evaluate member needs, and support care planning efforts. This position offers the opportunity to work independently while making a direct impact on patient outcomes and quality of care.
Key Responsibilities
- Conduct in-home and community-based nursing assessments.
- Evaluate members' physical, behavioral, and social health needs.
- Collect and document assessment findings accurately and timely.
- Identify barriers to care and recommend appropriate resources and interventions.
- Collaborate with care teams, providers, and community partners to support member care plans.
- Educate members and caregivers on health conditions, treatment plans, and available resources.
- Maintain compliance with documentation, quality, and regulatory requirements.
Qualifications
- Active Registered Nurse (RN) license in good standing.
- Minimum 2 years of clinical nursing experience.
- Strong assessment and critical thinking skills.
- Experience in case management, home health, managed care, community health, or care coordination preferred.
- Ability to work independently and manage a daily travel schedule.
- Valid driver's license and reliable transportation.
Preferred Experience
- Health risk assessments (HRA)
- Care management or utilization management
- Managed care or payer environment
- Community-based nursing or home visits
- Electronic medical record (EMR) documentation
