Position Summary

We are seeking a compassionate and experienced Registered Nurse (RN) for a 3-month contract assignment. In this field-based role, you will travel throughout the community to conduct health assessments, evaluate member needs, and support care planning efforts. This position offers the opportunity to work independently while making a direct impact on patient outcomes and quality of care.

Key Responsibilities

  • Conduct in-home and community-based nursing assessments.
  • Evaluate members' physical, behavioral, and social health needs.
  • Collect and document assessment findings accurately and timely.
  • Identify barriers to care and recommend appropriate resources and interventions.
  • Collaborate with care teams, providers, and community partners to support member care plans.
  • Educate members and caregivers on health conditions, treatment plans, and available resources.
  • Maintain compliance with documentation, quality, and regulatory requirements.

Qualifications

  • Active Registered Nurse (RN) license in good standing.
  • Minimum 2 years of clinical nursing experience.
  • Strong assessment and critical thinking skills.
  • Experience in case management, home health, managed care, community health, or care coordination preferred.
  • Ability to work independently and manage a daily travel schedule.
  • Valid driver's license and reliable transportation.

  • Health risk assessments (HRA)
  • Care management or utilization management
  • Managed care or payer environment
  • Community-based nursing or home visits
  • Electronic medical record (EMR) documentation

Locations include: Boston / North Boston / Lynn / Salem/Springfield / Holyoke (multiple openings - you would be responsible for 1)
