Resident Care Coordinator

We are seeking a compassionate and organized Resident Care Coordinator to enhance the experience of our residents and their families. This role serves as a key point of contact, ensuring residents feel welcomed, supported, and comfortable throughout their stay—from admission to discharge.

 

Schedule:

Monday–Friday | 8:00 AM – 4:00 PM (Full-Time) Occasional MOD weekend

Pay:

Based on experience and qualifications

 

Responsibilities include but are not limited to:

  • Ensure all open rooms are clean, staged, and move-in ready

  • Coordinate closely with Admissions, Marketing, and Social Services for upcoming admissions and discharges

  • Conduct routine rounds to address resident needs and requests

  • Maintain clean, organized, and welcoming common areas

  • Communicate effectively with residents and families, providing information or coordinating follow-ups

  • Conduct follow-up calls with families (“Caring Partner” calls)

  • Confirm required equipment (oxygen, wheelchair, etc.) is ready prior to arrival

  • Greet new residents and families upon admission

  • Assist with:

    • Inventory of belongings

    • Clothing labeling

    • Orientation to services and amenities

  • Help complete admission documentation with families

  • Coordinate discharge readiness and timing

  • Conduct a brief resident satisfaction survey

  • Coordinate post-discharge room turnover with housekeeping and admissions

Qualifications

  • High school diploma or equivalent required; associate degree preferred

  • Experience in healthcare, hospitality, or customer service preferred

  • Strong communication and interpersonal skills

  • Highly organized with attention to detail

  • Ability to multitask in a fast-paced environment

  • Compassionate, patient, and resident-focused attitude

Qualifications:

  • Previous experience in healthcare helpful, but not necessary.

  • Ability to work under stress and in emergency situations.
