Summary

The Physical Therapist provides comprehensive Physical Therapy services to patients referred by conducting assessments, developing and implementing treatment plans, and coordinating follow-up and discharge planning. This role actively contributes to the clinical aspects of the department, including program development, departmental operations, and performance improvement activities. Additionally, the Physical Therapist supervises and coordinates the training of Physical Therapy and Physical Therapy Assistant students, as well as overseeing Licensed Physical Therapy Assistants, Rehabilitation Technicians, and Volunteers.

Responsibilities
  • Conduct comprehensive diagnostic evaluations to assess the physical and cognitive abilities of inpatients and outpatients while considering age-specific needs.
  • Develop and implement individualized treatment plans with measurable goals, in collaboration with patients and their families.
  • Ensure compliance with documentation policies and procedures, including Initial Patient Evaluations, progress notes, discharge summaries, and other relevant records.
  • Participate in patient care conferences and departmental meetings, providing input and insights as needed.
  • Counsel patients and families regarding evaluations, care, and treatment while facilitating timely discharge planning and education.
  • Assist in the supervision and coordination of Physical Therapy and Physical Therapy Assistant student programs as well as volunteer efforts.
  • Support clinical operations by orienting new associates and managing patient scheduling to ensure appropriate coverage and supervision.
  • Demonstrate adherence to core values and engage in performance improvement initiatives.
  • Maintain knowledge of community resources to provide appropriate referrals as needed.
  • Identify educational opportunities for personal and departmental growth and participate in research activities as appropriate.
  • Ensure assessments, treatments, and care methods are tailored to meet the specific needs of each patient.
  • Foster positive and professional relationships with medical staff, physicians, and referral sources.
  • Perform related duties as assigned.
Requirements

Education/Skills

  • Bachelor’s, Master’s, or entry-level Doctorate in Physical Therapy from an accredited institution.
  • Familiarity with all modalities and equipment used in Physical Therapy practice.

Experience

  • No prior experience required.

Licenses, Registrations, or Certifications

  • Licensed as a Physical Therapist by the appropriate state licensing board.
  • BLS certification is mandatory.
