Case Manager

Assisted Living Facility in the Buffalo, NY region seeking a Case Manager

Salary: $63,000–$65,000 annually

Responsibilities:

• Assist with the integration of individuals into our senior living facility.

• Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to the facility, including addressing their social, emotional, and psychological needs while following DOH guidelines.

• Act as a liaison between residents, families, and other healthcare entities.

• Assist with quality assurance reviews and maintain accurate, up-to-date files and case management notes.

• Work closely with the entire care team to ensure a positive and safe environment for all residents.

• Demonstrate compassion and a strong commitment to working with the aging Education & Experience:

• Bachelor’s degree in a human services field and one year of full-time work experience in a related field; or

• Associate’s degree in a human services field and three years of full-time work experience in a related field.

• Previous case management experience required.

• Experience in Skilled Nursing, Assisted Living, and/or Memory Care required
