Clinical Liaison

A leading nursing and rehabilitation organization is seeking a Clinical Liaison to join its team. This position is ideal for someone with a clinical background who is also comfortable networking with hospitals, physicians, healthcare professionals, residents, and families.

As a Clinical Liaison, you will serve as an important connection between the healthcare community and the facility, helping identify appropriate referrals while providing education and support throughout the transition of care process.

What You'll Do
  • Develop and maintain strong relationships with hospitals, physicians, discharge planners, social workers, case managers, and other healthcare professionals

  • Identify and develop new referral opportunities within the community

  • Serve as a knowledgeable resource regarding skilled nursing, rehabilitation, and post-acute care services

  • Conduct facility tours and meet with prospective residents and families

  • Assist with the referral and admissions process by gathering and communicating clinical information

  • Collaborate with the admissions, nursing, therapy, and leadership teams to ensure smooth transitions of care

  • Follow up with referral sources and maintain ongoing relationships

  • Represent the organization at healthcare events, community meetings, and networking opportunities

  • Educate healthcare partners and families about available services and levels of care

  • Monitor referral trends and identify opportunities to increase census and strengthen community partnerships

  • Maintain accurate documentation and communication regarding referral activity

What We're Looking For
  • Clinical healthcare background preferred; RN, LPN, or other healthcare-related experience is a plus

  • Experience in skilled nursing, rehabilitation, hospitals, home health, hospice, or another post-acute healthcare setting preferred

  • Strong communication and relationship-building skills

  • Professional, outgoing, and confident personality

  • Ability to effectively communicate with healthcare professionals, residents, and families

  • Strong organizational and follow-up skills

  • Ability to work independently while collaborating with a multidisciplinary team

  • Knowledge of the healthcare referral and discharge planning process is a plus

  • Valid driver's license and reliable transportation may be required
