Regional MDS Coordinator

About Eminence

We are seeking a dedicated and detail-oriented Regional MDS Coordinator to join our team at Eminence. As a Regional MDS Coordinator, you will play a crucial role in ensuring the highest quality of care for our patients while fostering a positive and collaborative work environment.

Job Summary

The Regional MDS Coordinator is responsible for providing support to our facilities in the development and maintenance of accurate and thorough Minimum Data Set (MDS) assessments. This role requires strong analytical and organizational skills, as well as excellent communication and interpersonal skills. The successful candidate will work closely with our facility staff to ensure compliance with federal and state regulations, as well as company policies and procedures.

Responsibilities:

  • Conduct thorough and accurate MDS assessments for our patients, ensuring compliance with federal and state regulations

  • Work closely with facility staff to develop and implement individualized care plans and service plans

  • Maintain accurate and detailed records of MDS assessments, care plans, and service plans

  • Monitor and report on MDS assessment and care plan outcomes to facility staff and management

  • Collaborate with facility staff to identify and address potential issues related to MDS assessments and care plans

  • Stay up-to-date with changes in regulations and industry standards, providing training and support to facility staff as needed

Requirements:

  • Bachelor's degree in Healthcare Administration, Nursing, or related field (n/a)

  • Minimum of 5 years of experience in healthcare, with a focus on MDS assessments and care planning

  • Excellent analytical and organizational skills, with ability to maintain accurate and detailed records

  • Strong communication and interpersonal skills, with ability to work collaboratively with facility staff

  • Knowledge of federal and state regulations related to MDS assessments and care planning
