Responsibilities & Duties:
What you will own:
  • Maintain accurate patient and provider rosters; reconcile attribution and eligibility questions with authorized practice and ACO contacts.
  • Organize patient engagement and voluntary-alignment workflows using approved materials, preserving patient choice and tracking status through confirmed submission.
  • Maintain a calendar of applicable deadlines rather than assuming every program follows the same rules.

Make data useful:
  • Create prioritized outreach and care-gap worklists, support annual wellness and preventive-care planning, and help identify transition and utilization follow-up needs.
  • Review quality and utilization reports, distinguish data problems from care needs, and provide concise dashboards with clear owners and next steps.
  • Support accurate, clinically supported documentation and coding in partnership with providers and billing staff—not unsupported diagnosis capture.

Work with the team:
  • Coordinate with the physician-owner, practice leadership, nursing, scheduling, billing and the clinical-systems lead.
  • Clinical decisions remain with licensed clinicians.
  • Routine staffing and front-desk coverage are outside this role’s core assignment.
  • Use approved access, protect patient information and document processes so the work remains dependable when someone is away.

What you bring:
  • Demonstrated ACO, population health, quality improvement, care-management operations or value-based primary care experience.
  • A clinical, health administration, public health, business or related degree is preferred; relevant practical experience matters.
  • Strong spreadsheet/reporting skills, attention to detail and the ability to turn an uncertain data issue into a documented resolution are essential.
  • APRIMA familiarity and experience supporting older or medically complex populations are valuable.

Engagement:
  • Part-time hours, compensation, on-site needs and any remote component are agreed during recruitment. A fractional engagement may be appropriate for an experienced specialist.
  • Employee benefit eligibility depends on employment status, hours and plan terms.
  • Work directly with a physician-owner whose practice integrates adult primary care, geriatrics, behavioral health and serious-illness care.
