Physical Therapist (PT)

Key Responsibilities- Evaluate and treat patients to restore physical function across outpatient (90%) and inpatient (10%) settings
- Provide therapy services in General Orthopedics, Neurology, and Gait rehabilitation
- Establish individualized therapy goals and formulate treatment plans consistent with each patient's physical and functional status
- Document patient status accurately and communicate effectively with patients, families, and interdisciplinary treatment team members
- Supervise and assist therapy technicians and assistants as needed
- Doctoral degree in Physical Therapy (required for those licensed after 2016; those licensed prior to 2016 with a bachelor's or master's degree are grandfathered)
- Active Illinois Physical Therapy license, or confirmation that the application/licensure process has been initiated (IL License Waiver accepted)
- CPR certification required within 90 days of hire
- Must be able to produce a negative drug screen; position is located on a Drug-Free Campus
- Excellent interpersonal, communication, and analytical skills with strong computer proficiency including Microsoft software
- Candidates with a permanent address within 50 miles of the facility may be subject to a local rate adjustment
- Previous clinical experience as a Physical Therapist
- Valid state driver's license as verified through the respective state DMV driving record
- Demonstrated ability to evaluate patients' physical and functional status and adapt care to the population served
- Strong written, oral, and interpersonal skills with the ability to work collaboratively within a multidisciplinary team
- Work in a supportive critical access hospital environment with a diverse patient caseload spanning outpatient and inpatient settings
