Community Health Worker

· Deliver agency health education programs for target populations

· Develop relationships with community support services for collaborative outreach and referral

· Mentor and assist in training of community-based interns

· Update and translate presentations and/or handouts, or create new ones as needed

· Schedule services, events such as health fairs, presentations, and other activities in the community

· Conduct lung-health screenings, including spirometry, blood pressure and oxygen saturation screening

· Conduct clean indoor air quality home assessments (and fall prevention for seniors)

· Provide tobacco cessation information and assistance

· Distribute respiratory equipment and lung disease remediation supplies

· Prepare progress reports and program evaluation

• Resident of San Mateo, San Francisco, or East Bay preferred

Our mission is to fight lung disease in all its forms and to work with our community to protect lung health. We have a long history of serving our Bay Area communities with local programs that address important issues such as: COVID, vaping & smoking, asthma, COPD, lung cancer, tuberculosis, air pollution, wildfires and climate change.
