Job Summary
We are seeking a strategic and collaborative Director of Care Management to provide leadership and operational oversight for care management programs across hospital and ambulatory settings. This position will lead multidisciplinary teams, drive care coordination initiatives, improve patient outcomes, and optimize healthcare resources across the organization.
The Director will serve as a subject matter expert in care management and work closely with clinical leaders, community organizations, and cross-functional stakeholders to advance patient-centered care and organizational goals.
-> Key Responsibilities
- Provide strategic leadership and operational oversight for Integrated Care Management (ICM) programs across hospital and ambulatory settings.
- Lead multidisciplinary teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers.
- Develop strong partnerships with clinical leaders, community organizations, and internal stakeholders to promote seamless care integration.
- Drive process improvement initiatives focused on reducing readmissions, improving patient flow, optimizing length of stay, and enhancing care coordination.
- Monitor and evaluate clinical, operational, and financial performance metrics to support quality improvement and strategic objectives.
- Ensure compliance with applicable CMS, DNV, OSHA, OIG, and federal and state regulatory requirements.
- Coach, mentor, and develop team members while promoting accountability, collaboration, and service excellence.
- Allocate resources effectively and identify opportunities to remove operational barriers.
- Support organizational initiatives related to population health, care management, quality, and patient outcomes.
- Provide leadership and guidance to care management teams while promoting consistent practices across programs.
- Participate in a 24/7 leadership responsibility, including an on-call requirement.
- Coach and develop team leaders to strengthen decision-making and reduce the frequency of escalated calls.
-> Required Qualifications
Education:
- Bachelor's degree in one of the following:
- Bachelor of Science in Nursing (BSN)
- Bachelor of Social Work (BSW)
- Equivalent education in healthcare management
Licensure:
- Current, active license in good standing as either:
- Registered Nurse (RN), or
- Licensed Social Worker (LSW)
Experience:
- 3–5 years of progressive leadership experience in:
- Care Management
- Population Health
- Healthcare Management

- Experience leading multidisciplinary care management teams.
- Experience in hospital and/or ambulatory care management.
- Background in population health, utilization management, care coordination, or quality improvement.
- Experience working with healthcare regulatory and compliance requirements.
