About the Role:

As a Licensed Vocational Nurse (LVN), you will work under the supervision of a registered nurse or physician to provide direct patient care to our residents. You will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes. Responsibilities:

• Provide direct patient care to our residents, including administering medications, performing wound care, and assisting with daily living activities

• Assess patient needs and develop individualized care plans in collaboration with other healthcare professionals

• Implement care plans and provide ongoing patient education and support

• Collaborate with other healthcare professionals to ensure optimal patient outcomes

• Maintain accurate and detailed records of patient care and treatment

• Participate in quality improvement initiatives to improve patient care and outcomes

Requirements:

• Current LVN license in the state of Texas

• High school diploma or equivalent required; associate's degree in nursing (ADN) or diploma in nursing (DN) preferred

• One year of experience in a healthcare setting preferred
