Responsibilities:

• Assist our skilled nursing staff with providing direct patient care, including vital sign monitoring, bathing, dressing, and grooming

• Support residents with activities of daily living (ADLs), such as feeding, toileting, and ambulation

• Maintain a safe and clean environment by reporting any hazards or concerns to the nursing team

• Develop and implement individualized care plans to meet the unique needs of each resident

• Collaborate with other healthcare professionals to ensure comprehensive care coordination
