Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Part-Time and PRN  | 7am–7pm | 3 Days per Week + Every Other Weekend

Plum Creek Chicago s seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our care team. This role plays a vital part in delivering high-quality, person-centered care aligned with each resident’s individualized assessment and care plan — under the guidance of our Wellness Director.

• Provide daily nursing care and clinical services to residents.

• Perform health assessments and monitor vital signs.

• Partner with CNAs and care staff to identify resident needs and changes in condition.

• Administer medications and maintain accurate documentation.

• Offer clinical leadership and support to the care team.

• Foster a positive, resident-focused environment.

• Complete additional tasks as assigned by the Executive Director.

• Current Illinois RN or LPN license in good standing.

• Recent experience in a Memory Care or Supportive Living environment preferred.

• Strong clinical, assessment, and supervisory skills.

• Proficiency in medication administration and care documentation.

• High school diploma or GED.

• Completion of a state-approved nursing assistant program.

• Active status on the IDPH Health Care Worker Registry.

• Ability to lift up to 25 lbs and perform typical nursing and CNA support duties.

• Valid CPR certification (recently expired certifications accepted; recertification offered).
