Home Health Physical Therapist (PT) – Travel Assignment

Shift: Days – 8:00 AM–5:00 PM
Contract: 13 weeks
Schedule: Monday–Friday

Days: 8:00 AM–5:00 PM, Monday–Friday

Weekly Hours / Guaranteed Hours: 36 hours/week
Local Rule: Approximately 50-mile travel radius from Burlington, Iowa
 

Key Requirements

• Home Health PT experience – Required
• BLS
• Valid PT license – Required
• Able to work 36 hours/week
• Reliable transportation for home visits
