What You’ll Do:

As a Case Manager, you’ll play a vital role in coordinating care and supporting both clients and caregivers. Your responsibilities will include:

• Coordinating Starts of Care and Supervisory Visits
• Communicating with State Program and MCO Case Workers
• Supervising Personal Care Attendants
• Assisting with billing, scheduling, and office operations
• Collaborating with HR to maintain up-to-date staff files
• Providing direct personal care when needed

If you have experience with STAR+PLUS, STAR Kids, PHC, MDCP, or similar programs, we want to hear from you!

What We’re Looking For:

• High school diploma or GED (required)
• 2+ years of supervisory experience
• Experience managing PAS programs (preferred)
• Strong communication skills (Spanish-speaking a plus!)
• Comfortable with MS Word, Excel, and basic office equipment
• A heart for service and a commitment to quality care
