The Orchards is seeking a dedicated and knowledgeable MDS Coordinator to join our clinical team. This position plays a critical role in coordinating the Resident Assessment Instrument (RAI) process, ensuring accurate documentation, supporting individualized care planning, maintaining regulatory compliance, and helping maximize appropriate reimbursement.

The MDS Coordinator works under the direction of the Clinical Documentation Specialist and collaborates closely with nursing, therapy, physicians, residents, families, and the interdisciplinary team to support accurate assessments, effective care plans, quality outcomes, and exceptional resident-centered care.

What You’ll Do
  • Coordinate, schedule, and oversee MDS assessments, PPS documentation, Care Area Assessments (CAAs), and individualized care plans.
  • Coordinate assessment reference dates, scheduling changes, and completion deadlines to maintain compliance and support appropriate reimbursement.
  • Lead the interdisciplinary team in developing, reviewing, and revising individualized resident care plans.
  • Monitor Medicare certifications, skilled nursing utilization, and resident reimbursement to ensure documentation supports services provided.
  • Audit MDS assessments, CAAs, and care plans for accuracy, completeness, and regulatory compliance.
  • Complete or oversee admission, discharge, re-entry, and Medicare tracking documentation.
  • Monitor and evaluate Quality Measures and identify opportunities to improve resident outcomes and facility performance.
  • Implement and evaluate restorative nursing programs in collaboration with nursing leadership.
  • Attend and lead Medicare, Utilization Review, and interdisciplinary care planning meetings.
  • Serve as a facility resource for MDS regulations, Medicare reimbursement, PPS, Case Mix, RUG systems, and documentation standards.
  • Provide education and training to nursing staff and interdisciplinary team members regarding MDS requirements and documentation standards.
  • Communicate effectively with physicians, therapists, residents, families, and interdisciplinary team members regarding assessments and care planning.
  • Maintain accurate electronic documentation using the facility’s electronic medical record system.
  • Support nursing coverage and department operations as needed.
  • Promote teamwork, accountability, open communication, and The Orchards’ core values throughout the MDS process.
  • Perform other duties as assigned.
What We’re Looking For
  • Graduate of an accredited School of Nursing.
  • Current RN license in the state where the building is located, or eligibility to obtain licensure prior to employment.
  • Minimum of one year of nursing experience required; MDS coordination experience preferred.
  • Long-term care, skilled nursing, or geriatric care experience preferred.
  • Knowledge of Medicare, Medicaid, OBRA regulations, PPS, Case Mix, and Quality Measures preferred.
  • Strong assessment, organizational, analytical, and time-management skills.
  • Excellent communication, leadership, and teamwork abilities.
  • Ability to effectively communicate with residents, families, physicians, therapists, and interdisciplinary team members.
  • Proficiency with electronic medical records and Microsoft Office applications.
  • Ability to work independently while collaborating effectively with the interdisciplinary team.
  • Commitment to completing required annual state-mandated education and in-service training.
