🏥 **Travel Physical Therapist – Inpatient/Outpatient | Tuba City, AZ**
**⏳ Contract:** 12 weeks
**🕐 Schedule:** Monday–Friday | 8:00 AM–5:00 PM
**💼 Setting:** Inpatient & Outpatient

### 🩺 Requirements
* **3+ years of licensed PT experience**
* **Outpatient PT experience required**
* Bachelor’s degree from a **CAPTE-accredited PT program**
* Active, unrestricted U.S. PT license
* **AHA BLS/BCLS** required
* Must meet medical staff credentialing requirements
* **Fingerprinting must be cleared before start**

### 🧑‍⚕️ Responsibilities
* Evaluate and treat **inpatient and outpatient** patients from birth through geriatric
* Provide PT services for prevention and treatment of physical disabilities/disease
* Specialty services including:
* Wound care
* Prosthetic & orthotic clinic
* Geriatric and high-school clinics
* Spinal cord and functional clinics
* Adaptive equipment & assistive technology
* DME/prosthetic/orthotic services
* Support quality improvement, safety, staff education, policies, and procedures
* Participate in weekend rotation as needed

* Orientation occurs every 2 weeks
* COVID & flu exemptions may be accepted with required declination form
* Housing application must be submitted **2 weeks before start**
* No on-call requirement
* W-2 only; no 1099
