Community Health Nurse Supervisor

THE POSITION

DESCRIPTION OF WORK

This role focuses on leading public health services, supervising staff, and coordinating efforts that improve community health across assigned counties. You will support disease investigations, strengthen partnerships, and guide clinical practices within the Bureau of Community Health Systems. As a Community Health Nurse Supervisor, you will perform the following duties:

• Disease Oversight: Ensure surveillance and case investigations for reportable diseases using approved systems and manuals
• Clinical Direction: Guide clinical practices and public health services in line with bureau policies and procedures
• Community Assessment: Lead assessments to identify health needs and determine priorities across assigned counties
• Staff Leadership: Provide direction, consultation, and orientation to nursing and clerical staff to maintain a competent workforce
• Partner Coordination: Work with agencies, physicians, hospitals, and schools to support public health initiatives and outreach
• Program Monitoring: Track community health programs using public health information systems to improve health outcomes

Work Schedule and Additional Information:

• Full-time employment, 37.5 hours per week
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. Weekend and after-hours assignments are required as needed.
• Headquarters: The location will be determined by the selected candidate and may be at the state health center in Cumberland, Juniata, or Mifflin County.
• This position supervises Community Health Nurses and clerical staff within the Southcentral District which includes the following counties: Adams, Bedford, Blair, Cumberland, Dauphin, Franklin, Fulton, Huntingdon, Juniata, Lebanon, Mifflin, Perry, and York.
• Telework: You may have the opportunity to work from home (telework) part-time. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the selected headquarters office. The ability to telework is subject to change at any time. Additional details may be provided during the interview.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

• Three years as a Community Health Nurse (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration); or
• Four years of community health nursing experience; or
• An equivalent combination of experience and training.

Special Requirements:

• This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

Legal Requirements:

• You must pass a background investigation.
• This position falls under the provisions of the Child Protective Services Law.
