Long Term Care Coordinator (LTC Coordinator) – Utilization / Authorizations

Location: Remote (Alameda, CA / California preferred)
Schedule: Monday–Friday, 8:00am–5:00pm PT
Pay: $40.02–$60.03/hour


Summary


Hiring a Long Term Care Coordinator to support long-term care clinical teams by managing high-volume authorization and documentation workflows across the continuum of care. This Long Term Care Coordinator role is heavily focused on processing, tracking, and monitoring LTC admissions, transitions, bed holds/leave of absence requests, and service coordination. Responsibilities


  • Coordinate and process Long-Term Care admissions and transitions (SNF / facility coordination)
  • Monitor the review and authorization workflow; ensure required documentation is received on time
  • Issue member/provider notifications (mail, fax, electronic, phone)
  • Coordinate supportive services (DME, transportation, therapies)
  • Use established Title 22 guideline pathways for screening/authorizing LTC requests (bed holds, LOA)
  • Run scheduled and ad hoc reporting on utilization/authorization status; identify trends
  • Support inquiries related to claims, authorizations, appeals, eligibility, complaints/grievances intake
  • Perform accurate data entry across multiple systems; maintain audit-ready documentation


Qualifications


  • 1+ year experience in a managed care or healthcare setting (preferred)
  • Experience with Medi-Cal and Medicare populations and related regulatory requirements
  • Knowledge of DMHC regulations and DHCS contractual requirements (preferred)
  • Strong data entry, organization, and multitasking skills in a fast-paced environment
  • Proficiency with Microsoft Office (Word/Excel/Outlook) and Windows-based systems
  • Excellent written and verbal communication skills
  • Valid driver’s license and auto insurance (required)
