Job Description
A leading Brooklyn community hospital seeks a Nurse Manager, Case Management to lead and oversee the daily operations of the hospital's Case Management Department. The Nurse Manager will be responsible for ensuring effective care coordination, utilization management, discharge planning, and regulatory compliance while driving positive patient outcomes and efficient resource utilization. This leader will supervise case management staff, collaborate with multidisciplinary teams, and promote best practices in patient-centered care.
Essential Responsibilities
- Manage the day-to-day operations of the Case Management Department, including case managers, social workers, and support staff.
- Provide leadership, coaching, performance management, and professional development for the case management team.
- Ensure compliance with CMS, Joint Commission, NYS Department of Health, and other regulatory requirements.
- Oversee utilization review activities to ensure appropriate level of care and optimize reimbursement.
- Lead discharge planning processes to facilitate timely, safe, and effective transitions of care.
- Collaborate with physicians, nursing leadership, ancillary departments, and community resources to coordinate patient care.
- Monitor key performance indicators including length of stay, readmission rates, denial management, and patient throughput.
- Participate in interdisciplinary rounds and care coordination meetings.
- Develop and implement policies, procedures, and quality improvement initiatives.
- Ensure accurate and timely documentation within the electronic medical record.
- Assist with department budgeting, staffing, scheduling, and resource management.
- Serve as a clinical and operational resource for complex patient care situations.
Qualifications
- Bachelor of Science in Nursing (BSN) required.
- Current New York State Registered Nurse (RN) license in good standing.
- Certified Case Manager (CCM) certification preferred.
- Minimum of 5 years of acute care hospital case management experience.
- Minimum of 2 years of leadership experience within an acute care setting.
- Strong knowledge of utilization management, discharge planning, care coordination, and payer regulations.
- Experience working with Medicare, Medicaid, commercial payers, and managed care organizations.
- Excellent communication, leadership, organizational, and problem-solving skills.
Salary: $150,000 - $180,000
