What You'll Do
- Provide skilled physical therapy services to residents in a SNF setting.
- Perform evaluations and develop individualized treatment plans.
- Help patients improve strength, balance, mobility, and functional independence.
- Assist patients with recovery following illness, injury, surgery, or hospitalization.
- Collaborate with nursing, physicians, and the interdisciplinary rehab team.
- Complete accurate and timely clinical documentation.

Requirements
- Active North Carolina Physical Therapist license or eligibility to obtain.
- SNF/LTC experience preferred.
- Current BLS certification.
